Empty (white/transparent) Quick Access Toolbar - QAT.

A - answer AutoCAD in Windows displays the top Quick Access Toolbar only in white - the icons are almost transparent and illegible:

This behaviour means that graphics acceleration in your Windows driver doesn't work properly. Try to update your Windows graphics driver or change its display settings for Windows (level of 3D acceleration, color depth, transparency, text size, etc.). In Windows 7 you can also try to switch between the Aero/Classic environments.

This should repair the QAT appearance.
