How to convert a .PLT file back to .DWG?

A - answer You can never perform a 100% reconstruction of a plot file (.PLT,.PRN) back to a CAD drawing - the PLT files do not contain full information about the individual original entities.

For visual information you can use one of the view/conversion utilities. E.g. if your PLT file is in the HPGL/2 format you can use HPGL2DXF, UNPLOT, SPLOT or other CAD utility.
