hi all,
is there a way of framing the top line of text only on multileader? - circles/ovals or squares, anything will do!

I assume the first line of the multiline text is the one in the bubble?

yep - I just had to make a new paragraph and then manually draw circle around it....

I couldn't come up with a way to do it.  You could always use the dynamic tag block that comes with AutoCAD for the number in the circle then add the text below using multiline text.  Another option would be to create your own dynamic block.
