I have taken a measurement off an HVAC diagram, the diagram says 12" and the measured distance is 1/8 of an inch. I realize that meens 0.125"=1' but as all the measurements are different in the diagram I am having a hard time figuring out the easiest way to put this into autocad. I think I'm just having a brain fart

That scale can also be expressed as 1"= 8 feet or 1"=96 inches.
It would help if you had a 1/8" scale to take measurements off the drawing.
You can choose to use inches or feet units when you draw in AutoCAD.
