This error message is caused by an attempt to create block of the same name as the block already contained (nested) in the block you insert.
For example, if you have an empty drawing, you create a new block "ABCD" in it and then you save the whole drawing to a file ABCD.DWG, any attempt to insert this DWG (as a block) to another drawing will invoke this error message. The proper procedure for this was to save your block to a DWG file with WBLOCK.
This nam collision may also appear on deeper nesting levels (a block in a block in a block).
