You can edit the ACAD.MNS file (located in the folder specified in ROAMABLEROOTPREFIX variable) and add your new commands item to the ***POP501 (no object selected) and ***POP502 (object selected) sections. Save the ACAD.MNS file and then restart AutoCAD.
