The REFEDIT command reports:

Errors found in references to other objects:
** Application reference missing: ACAD, to AcDbBlockTableRecord.
(or AcDbLinetypeTableRecord)

Perform an AUDIT of the main and the reference (block) drawing. The DWG file(s) is probably corrupted, or it may be improperly saved back from a higher version, or an object enabler may be missing, or it may come from a non-Autodesk CAD application (a faked DWG format file). Check also possible nested xrefs of the xref.
