we found it: acad/ vault tab/ vault options/ prompts/ in command column, on 'check out', Some properties may be out of date. Do you want to update properties now? Affected file(s): {0} he had it set under 'frequency' to 'never prompt'...which is why we (I) didn't have problems and this individual did. We changed that setting to 'always prompt'. Solved for now! but I have a feeling the solution was more 'work around' than permanent fix...

Just did that. apparently vault is pushing back user defined attributes into the drawing title block...why is the question now

OK....so this only happens on this one computer.
Well scratch that idea (re: attribute).
Have you tried stripping out the block, purging the drawing, then inserting a new block?
